Southcentral Foundation’s Nuka System of Care can help your organization engage your patients (called “customer-owners”) and the community in ways that will lead to better outcomes for your organization and the community. Learn how to make customer and community engagement a priority across your health care system.

The use of data to drive improvement is a key tenet of Southcentral Foundation’s Nuka System of Care. Our unique population-focused, person-centric data management tool is designed to share up-to-date data with physicians and care teams – allowing them to identify and engage high-risk patients to ensure they receive needed care. A person-centric approach has ensured that patients (called customer-owners) are accessing the appropriate level of service, impacting long-term health outcomes and increasing satisfaction.

The health care industry is undergoing rapid change. While improvement work is often linked to a specific project or program, fostering an organization-wide improvement culture can have long-term impacts on health outcomes and customer satisfaction. Southcentral Foundation’s Nuka System of Care (Nuka) is structured for continuous improvement using the voice of the patient (called “customer-owners”) and grounded in the Malcolm Baldrige National Quality Award framework.

SCF’s Nuka System of Care is customer-driven, with customer-owners shaping every aspect, including facility design. Through a four-phase redesign process, SCF gathered feedback at each stage, making improvements that enhanced clinic functionality and secured community buy-in, a practice that continues with new facility projects.
