Training for Integrated Care Teams

As previously discussed on this blog, Southcentral Foundation’s Integrated Care Teams build strong relationships with customer-owners in order to provide more effective care which addresses their unique health needs and helps them achieve overall wellness. In order to support care teams in offering this kind of relationship-based care, SCF provides extensive training to teach care workers the skills they need.

Training for Integrated Care Teams

In addition to the dedicated Integrated Care Team training SCF provides, SCF offers many other trainings that help ICT members with various aspects of their jobs. One example of this is SCF’s Core Concepts training, but many other trainings are also provided. Following is information about training for Integrated Care Teams.

5 Dynamics

5 Dynamics is a tool that SCF uses to help work teams optimize individual and team performance. The tool utilizes an assessment and workshops to understand how individuals and teams prefer to get work done.

The training is offered as both an individual and group workshop. The individual workshop examines how differing energies impact the work cycle, employee learning preferences, and how employees can apply the right energy at the right time to move through the work cycle effectively. The group workshop helps teams develop a deeper understanding of each other and their team as a whole, identify the benefits and challenges posed by their team’s dynamics, and develop ways to align their team’s preferred processes with the process demands of their team.

Difficult Conversations

This training helps to create alignment and agreement by fostering open dialogue around high-stakes, emotional, or risky topics within a health care setting. The training teaches participants how to review the elements needed for an effective difficult conversation, discuss their style under stress and how it relates to effective conversations, and explore how difficult conversations can contribute to burnout or compassion fatigue.

Compassion Fatigue

Compassion fatigue is something that can sometimes be experienced by those helping people in distress. This training teaches participants how to define compassion satisfaction, compassion fatigue, and burnout; how to examine the signs and symptoms of compassion fatigue and burnout; how to understand the survival triangle and how engaging in it can lead to compassion fatigue; and how to develop tools to help prevent or recover from compassion fatigue and burnout.

Physician Engagement

Physician engagement is the process of bringing physicians together with other healthcare stakeholders to continuously improve care and the patient experience. SCF encourages physicians and providers to engage their customer-owners as well as their team members. This training teaches participants how to define and apply physician engagement as it is used at SCF, and how physician engagement has increased overall satisfaction rates and improved healthcare outcomes.
